Congestive Heart Failure

Better known as CHF, this disease results in a weak heart and fluid retention.

What is CHF?

CHF stands for congestive heart failure. It is a disease in which the heart’s pumping power is weaker than normal, meaning that your blood moves more slowly through your body. Your heart compensates for this by stretching to try and hold more blood or by becoming stiff and thickened. This also causes the kidneys to retain more fluid, which builds up in your lower extremities such as feet or ankles.

CHF can be caused by coronary artery disease, a heart attack, infections from alcohol or drug abuse, high blood pressure, valve disease, diabetes, and many other problems. It is a disease which currently affects six million Americans, with 670,000 new cases being diagnosed each year. Unfortunately, it is the leading cause of hospitalization for people over the age of 65. Luckily, however, we have developed a strong and reliable program to care for, educate, and monitor any of our patients with CHF.

What is our CHF Program?

Gallagher Home Health Services’ CHF Program helps us manage our patients with cardiovascular disease so we can keep them out of the hospital. We combine the highest standards of care with advanced technologies such as TeleHealth Monitoring to give our patients the best possible outcome.

We follow a strict set of guidelines to monitor our patients’ conditions. We measure fluid output, manage medications, determine daily weight records to determine how our patients are doing. In addition, some of our patients are eligible for TeleHealth monitoring. Our monitor measures a patient’s heart rate, blood oxygen level, blood pressure, and weight each day. This data is sent directly to a nurse at our office, who watches the patient’s data each day and alerts us to intervene when necessary.

We are experts at educating patients on the signs and symptoms of CHF as well as how to care for themselves. Under our care, patients can learn about their disease from the comfort of their own home. Additionally, the data we collect paints the real picture of how the patient is managing their care at home.
